King-Beyond-the-Wall is the name given to a person who united the many tribes of wildlings under his authority. The term seems to have been invented by the people of the Seven Kingdoms. Despite its name, the title is not necessarily a hereditary or absolute position. It is a position granted by the wildlings based on merit and need. For long periods of time there can be no recognized king. The most recent King-Beyond-the-Wall, Mance Rayder, competed against multiple other contenders, bribing and bargaining with the many tribes to recognize him as their leader.
